Please give us a call or send us a photo of your insurance card prior to your appointment.
In addition, we accept all major credit cards, checks and cash.
Payment plans may be set up through Care Credit. Be sure to ask about all the details during your visit.
Payment is due at the time of service.
You will be informed before treatment what your ESTIMATED patient portion will be.
The ESTIMATE will be based on the insurance information you have shared with our office.
You are responsible for any amount that is above or beyond the ESTIMATED
patient portion that your insurance company does not cover.
All remaining balances on your account are to be paid in full within 60 days of treatment.
